COVID-19
COVID-19 is a respiratory infectious disease caused by the novel coronavirus (SARS-CoV-2), characterised by fever, cough, dyspnoea (shortness of breath), and in severe cases, pneumonia and multi-organ failure requiring intensive care support. The pandemic emergence in late 2019 subsequently spread globally becoming a major public health and economic crisis affecting healthcare systems across Pakistan and worldwide.
Clinical manifestations span asymptomatic infections through mild upper respiratory symptoms (fever, cough, sore throat) in majority of infected individuals (approximately 80%), to severe lower respiratory tract involvement requiring mechanical ventilation in approximately 5% of cases. Age >65 years and comorbidities including diabetes, hypertension, and chronic lung disease significantly increase severe disease and mortality risk. Transmission occurs through respiratory droplets and aerosol pathways requiring social distancing and ventilation controls minimising transmission.
Diagnostic confirmation employs RT-PCR (reverse transcription polymerase chain reaction) testing detecting viral RNA, with antigen rapid testing providing faster results at point-of-care. Serologic testing detecting antibodies indicates prior infection or vaccination response.
Therapeutic approaches include supportive care (oxygen, fluid resuscitation, mechanical ventilation), monoclonal antibody infusions reducing viral load in early disease, and immunomodulatory agents (corticosteroids, IL-6 inhibitors) reducing excessive inflammatory responses in severe cases. Vaccination programmes employing mRNA, viral vector, or inactivated virus vaccines provide substantial protection against severe disease reducing hospitalisation and mortality.
