The Philadelphia Tracheotomy Collar is a cervical immobilization device modified specifically for patients who have undergone tracheotomy procedures, providing cervical spine support while maintaining unobstructed access to the tracheotomy stoma and tube assembly on the anterior neck. Standard cervical collars cover the anterior midline neck surface where the tracheotomy tube exits, making them incompatible with tracheotomy patient care without modification. The Philadelphia Tracheotomy Collar resolves this through a purpose-designed anterior shell with a large central cutout sized and positioned to clear the tracheotomy tube, ties, and dressing area entirely.

The anterior shell uses rigid polyethylene construction with the tracheotomy access opening measuring approximately 60 to 80 millimeters in diameter, centered at the level of the second and third tracheal ring position to align with standard surgical tracheotomy placement sites. Open-cell foam padding lines the inner surfaces of both anterior and posterior shells, providing cushioning against skin pressure during extended inpatient wear without covering or compressing the tracheotomy site margins. Additionally, the posterior shell maintains standard Philadelphia Collar geometry with full occipital support and bilateral lateral walls that restrict extension and rotation without modification to the posterior component.

Lateral hook-and-loop closure straps connect the two shells on both sides of the neck, with adjustment range covering standard adult neck circumferences from small through extra-large without requiring separate sizing hardware. Furthermore, the foam padding material uses moisture-resistant open-cell construction that resists secretion absorption from tracheotomy site drainage, simplifying hygiene maintenance and reducing skin maceration risk at the collar contact surfaces during critical care or extended ward admission periods.

Radiolucent shell material allows cervical imaging without collar removal during monitoring of the underlying injury or surgical condition requiring immobilization. Moreover, the collar withstands disinfection with standard hospital-grade cleaning solutions between patient uses without degradation of the shell or foam components.
