Description:
A medical traction splint is an immobilization device that applies controlled longitudinal pulling force to a fractured limb, typically the femur, to realign bone fragments, reduce muscle spasm-induced shortening, and relieve pain during patient transport to definitive surgical care. Femur fractures cause powerful surrounding muscle contraction that pulls fractured bone ends together with significant overlap and angulation, and traction splinting counteracts this contraction by applying steady distal pull, restoring approximate bone length and alignment while immobilizing the limb for transport.

The splint consists of a rigid pole or telescoping rail extending along the length of the leg, an ischial or groin strap anchoring the proximal end of the device against the pelvis to provide a counter-traction point, an ankle hitch or harness securing the distal foot and ankle, and a ratchet or windlass mechanism that the operator turns to apply and adjust traction tension progressively. The Hare traction splint and Sager traction splint represent the two most widely used designs, differing primarily in their counter-traction anchor point and ratchet mechanism configuration. The Hare splint uses an ischial strap against the pelvis with an external pole running along the outside of the leg, while the Sager splint uses a perineal strap with a single medial pole and a simpler ratchet adjustment mechanism.

Application requires the patient to be supine with the injured leg in a neutral position. The provider applies the ankle hitch, extends the splint to appropriate length, secures the proximal anchor strap, and progressively applies traction through the ratchet mechanism while monitoring distal pulse and sensation to confirm adequate but not excessive tension.
